Who can use this form
Use this form to request academic or physical accommodations for your Continuing Education courses at Wrenford Polytechnic. This applies to part-time and online learners who need changes to how they participate in class or complete their assignments. You can submit a request even if you have not previously accessed student supports.
Types of accommodations
Academic accommodations might include extra time on assignments, access to lecture recordings, or permission to take exams in a quiet room. Physical accommodations could involve adjusting the layout of a workshop, providing ergonomic seating, or allowing breaks during long practical sessions. We work with you to find solutions that fit your specific needs while keeping the learning outcomes of the course intact.
How to submit your request
- Log in to the student portal and open the Accommodation request form.
- Fill in your personal details and the course code for the class you need support with.
- Describe the accommodation you are asking for. You do not need to share your full medical history. A summary of how the barrier affects your learning is enough.
- Attach any supporting documents you have, such as a doctor's note or an assessment report.
- Submit the form. You will receive an automated email confirmation within one business day.
What happens after you submit
Once your form is submitted, a member of the Accessibility Services team will review your request. We may contact you to discuss your needs in more detail. If we need additional information, we will send you an email with a list of questions. Please reply as soon as you can so we can move forward with your plan.
How long does it take?
We aim to respond to every request within five business days. If your request is approved, you will receive an accommodation plan outlining the supports you will receive. This plan will be shared with your instructor so they know how to help you in class. You do not need to tell your instructor about your personal health information. They only need to know the accommodations you are approved for.
Deadlines and fees
There is no fee to submit an accommodation request. We process requests on a first-come, first-served basis. Please submit your form at least two weeks before the course start date to give our team enough time to arrange your supports. Requests submitted later may still be approved, but we cannot guarantee the accommodation will be ready before your first class.
Changes to your accommodation
If your situation changes or you need a different accommodation for a new course, you must submit a fresh request using this same form. The team will review the new details and update your plan accordingly.
