Submit an incident report
Use this process to submit an incident report for accidents occurring during athletics or recreation activities. Reporting helps us keep our facilities safe, process any applicable insurance claims, and arrange academic accommodations if you need extra time for assignments or exams.
Who this covers
This report applies to any student, staff member, volunteer, or visitor who sustains an injury or is involved in an accident while participating in a Wrenford Polytechnic sports team, intramural league, fitness centre session, or other recreation program.
How to report an injury
Follow these steps as soon as you are able:
- Call for help if needed. If the injury is severe, call 911 immediately. For non-emergency injuries, visit the Health Centre or see a campus nurse.
- Notify Campus Security. Campus Security incident logging is the first point of contact for on-campus accidents and near-misses. You can call the security line or visit the desk in person.
- Complete the Injury and Illness Investigation Form. You will find this form on the Campus Security website or through the myWrenford portal. Fill out all required fields, including the date, time, location, and a brief description of how the injury happened.
- Inform your coach or instructor. Let your program leader know so they can adjust your training schedule and track any patterns of injury in their classes or teams.
Deadlines and costs
Reports must be submitted within 24 hours of the incident. This deadline is required for insurance processing and academic accommodation approval. There is no fee to file a sports injury report.
Support and follow-up
Recovering from a sports injury can be stressful. Wrenford Polytechnic offers free confidential counselling sessions to help you manage the mental side of recovery. If you are an international student, your mandatory student health plan may cover additional medical visits. Speak with your instructor about adjusting deadlines or lab work while you heal.
