Report an injury on campus or during a work term
This page covers the procedure for documenting workplace or campus injuries for insurance and academic accommodation purposes. If you are hurt while studying at Wrenford Polytechnic or completing a work term, follow these steps to protect your health, your insurance coverage, and your grades.
Who this is for
This process applies to any student injured on campus property, in a college vehicle, or during a supervised work term, practicum, or co-op placement. It covers minor injuries, accidents, and near-misses that require documentation.
What to do right away
Your health comes first. If you need medical care, visit the Health Centre on Benchlands Campus or call 911 for an emergency. Once you are safe, follow these steps to file your report.
How to file a report
- Report the incident to Campus Security. Call 911 for emergencies, or contact Campus Security at 519-555-0176 to log the accident. They will send an officer to the scene or meet you at the Health Centre.
- Complete the incident report form. A Health Centre staff member, a First Aid Attendant, or your workplace supervisor will provide the Injury and Illness Investigation Form. Fill it out completely, or have your instructor or supervisor complete it for you. You must submit the form within 24 hours of the incident.
- Submit the form for insurance. Send the completed form to Campus Security or your employer's health and safety representative. This step is required for your insurance coverage and any related claims.
- Request academic accommodations. Bring a copy of the completed incident report to the Registrar office in Hollis Hall. The Registrar will review your documentation and arrange a temporary course load adjustment or make-up work plan so your grades are not affected while you recover.
- Report work-term injuries separately. If you were injured during a co-op placement or unpaid practicum, also notify Co-op & Career Services. They will coordinate with your employer to ensure the incident is recorded for insurance purposes.
Fees and deadlines
There is no fee to report an injury or to file an incident investigation form. You must submit your report within 24 hours of the incident. Late reports may delay insurance processing and academic accommodation approval.
