Immunization requirements

Who needs this

If you are enrolled in a Health Sciences program, you must submit your immunization records before you can start classes and before you begin clinical placements. This applies to all students in Practical Nursing, Medical Laboratory Technology, Medical Radiation Technology, Pharmacy Technician, and other health programs.

How to complete your health clearance

Follow these four steps to make sure your records are up to date.

Step 1: Gather your records

Find your personal vaccination history. You will need to complete the Student Health Clearance Form. Write down every vaccine you have received, including the date and the name of the vaccine. If you do not have your records, ask your family doctor, public health unit, or pharmacy. You can also check your provincial health portal if it stores immunization history.

Step 2: Fill out the form

Download the form from the Health Centre page or pick up a copy at the Health Centre on the Benchlands Campus. Fill in all sections you can. If you are missing information, leave those boxes blank and mark them as pending. You do not need to get any extra shots or blood tests before you hand in the form.

Step 3: Submit your documents

Scan or take clear photos of your completed form and any supporting documents. Send them by email to [email protected] or upload them through your myWrenford student portal. Use your official student email address when you send the message. Include any questions in the body of the email.

Step 4: Review and follow-up

A nurse at the Health Centre will review your paperwork within five business days. You will receive an email confirming receipt and listing any missing items. If you need a blood test to prove immunity to certain diseases, the Health Centre will send you a lab requisition. You can take this requisition to any local laboratory. If you need additional vaccines, the clinic will give you a schedule to complete them before your placement starts.

Deadlines and costs

Deadline: You must submit your completed health clearance package by September 1, 2026 for the Fall intake. Spring and Winter intake students must submit their forms by January 15, 2027 and May 1, 2027, respectively. Missing the deadline may delay your access to clinical sites.

Costs: There is no fee to have your immunization records reviewed. The Health Centre does not charge for this service. All vaccines required for your program are publicly funded for Ontario students. If you require a TB skin test, you will need to book it through a local provider, and you will pay the provider directly. Some students may need a criminal reference check and a drug screening before starting shop or placement sessions; those costs are handled separately.

Questions or concerns

If you cannot find your records, need help filling out the form, or have questions about specific vaccines, contact the Health Centre nurses. We can help you figure out the best next steps. You can also book a quick visit to see a nurse at the Health Centre on the Benchlands Campus.
