Accessibility support forms

Download and print these forms

Accessibility Services provides downloadable forms to help you request academic accommodations, housing adjustments, or medical documentation. You can use these forms if you have a physical, learning, mental health, or chronic health condition that affects your studies or campus life.

Available forms

  • Medical documentation form — For students who need to provide professional verification of a condition to approve accommodations.
  • Housing adjustment request form — For students who need a different room type, a roommate change, or accessibility modifications in residence.
  • Exam accommodation request form — For students who need extra time, a separate room, or assistive technology during assessments.

How to submit your forms

  1. Download the correct form from this page.
  2. Print the form and fill it out completely. If you prefer, you can fill it out digitally and attach it as a file.
  3. Submit the form to the Accessibility Services office in Hollis Hall, Room 210. You can also email a scanned copy to [email protected].
  4. Book an intake meeting with Accessibility Services to review your request and confirm your accommodations.

Deadlines and fees

There is no fee to register with Accessibility Services or to receive academic accommodations. We ask that you submit your completed forms and any supporting documentation before the term start date. For exam accommodations, you must submit your request at least 10 business days before your first exam of the term. Late requests may still be accepted, but accommodations are not guaranteed until the file is complete.
